Physician - Geriatric Care - PACE Program

Fallon Health is seeking a Physician specializing in geriatric care for its Summit ElderCare® PACE program in Leominster, MA. This role offers the opportunity to provide individualized quality care as part of an interdisciplinary team focused on helping older adults live safely at home. Physicians enjoy the flexibility to dedicate time as needed for clinical and social needs of participants and work collaboratively within a diverse healthcare team.

• Provide primary and specialty care for older adults.
• Collaborate with an interdisciplinary team including therapists, dieticians, nurses, and social workers.
• Develop and implement care plans that address the specific needs of participants.
• Maintain detailed patient records and contribute to the continuous improvement of the program.

• Medical degree with a specialization in Geriatrics, Internal Medicine, or Family Practice.
• Licensed to practice medicine in Massachusetts and holding a current DEA certificate.
• Board certification in Internal Medicine, Geriatrics, or Family Practice preferred.
• Minimum two years of experience working with a frail or elderly population; management experience is desirable.
• Compliance with Fallon Health’s COVID-19 vaccination requirements.
