Nurse Practitioner - Geriatric care

Fallon Health is seeking a Nurse Practitioner specialized in geriatric care to join their Summit ElderCare team in Leominster, MA. This role focuses on delivering personalized, comprehensive care to the elderly using an interdisciplinary model. The candidate will work closely with a team to manage a small caseload, ensuring quality over quantity in patient interactions.

• Provide primary and specialty geriatric care as part of an interdisciplinary team.
• Conduct assessments, develop care plans, and manage the social and clinical needs of patients.
• Engage in family support and caregiver education programs.

• Master’s degree and graduation from an accredited Nurse Practitioner program required.
• License to practice in Massachusetts and experience with geriatric care preferred.
• Strong skills in care coordination and ability to work flexibly in a dynamic environment.
• COVID-19 vaccination required for non-remote roles as per company policy.
