Aging Services of North Central Massachusetts is seeking an Adult Foster Care Registered Nurse (RN) to work collaboratively with the Care Manager to conduct assessments and develop care plans for consumers in the Adult Foster Care program. The role involves assessing consumer and caregiver needs, developing and implementing care plans, and maintaining records. The RN will provide orientation, monitor health status, and ensure high-quality care aligned with agency values.

Responsibilities

  • Conduct initial and ongoing assessments of consumers, caregivers, and homes.
  • Develop and regularly review the Adult Foster Care plan of care.
  • Provide orientation and oversight to caregivers and LPN staff.
  • Monitor the health status of all consumers and maintain case records.
  • Participate in discharge planning, case discussions, and staff meetings.
  • Adhere to program standards and represent the agency professionally.

Job Requirements

  • Current RN license from the Massachusetts Board of Registration in Nursing.
  • Experience with elderly populations and/or community health.
  • Strong communication, organization, management, and nursing assessment skills.
  • Knowledge of medical, mental health, and social service delivery systems.
  • Valid driver's license and legally insured and registered motor vehicle.
  • Physical ability to perform job functions, including visiting consumers' homes in various weather conditions.
