Aging Services of North Central Massachusetts is seeking an Adult Family Care LPN to join their team. This role involves conducting initial and ongoing assessments, developing care plans, and monitoring the health status of consumers in the Adult Foster Care program. The position requires collaboration with care managers and registered nurses to ensure high-quality care and support for elderly individuals and their families.
Responsibilities
- Conduct initial and ongoing assessments of program participants, caregivers, and homes.
- Develop, implement, and review care plans in conjunction with the care team.
- Provide orientation and training to caregivers.
- Monitor the health status of all consumers.
- Maintain accurate and up-to-date case records and documentation.
- Participate in case discussions, discharge planning, and other meetings.
Job Requirements
- Current LPN license from the Massachusetts Board of Registration in Nursing.
- Experience and interest in working with the elderly population or in community health.
- Strong communication, organization, assessment, and documentation skills.
- Ability to work under supervision from RNs and a Clinical Nurse Director.
- Valid driver's license and access to a legally insured and registered vehicle.
- Physical ability to navigate uneven terrain, climb stairs, and manage occasional lifting up to 25 pounds.
